| 1. Using a pair of scissors, straight edge, safety or electric trimmer cut the hair strands at the skin or scalp level. |
| 2. Collect a group of hair strands, full length about a 1/4″ in diameter if held between your finger and thumb from the scalp. Roots or follicles are not required. |
| 3. Place the strand sample in a clean, white, mailing envelope with the sample donors name, address, phone number, and email printed on the front. |
| 4. Print out and complete your personal health questionnaire for submission with your test sample. |
